Service Description
If you are a patient of Dr. Gray or Dr. Ajagbonna and have seen your family physician within the last 6 months, you can call your pharmacy and request a "Fax Refill Request" to be sent to our office for a signature (no appointment needed). If you are a patient of Dr. Motsiri and/or have not seen your family physician in over 6 months, please proceed with booking an appointment.
Cancellation Policy
We require 24 hours notice for appointment cancellations (1 business day). There is a fee of $50 for a missed appointment or appointment cancelled on short notice. Patients who repeatedly miss appointments will be discharged from the practice.
